About

Aprepitant is a selective high-affinity antagonist of human substance P neurokinin 1 (NK1) receptors. It is primarily used in combination with other antiemetic agents, such as corticosteroids and 5-HT3 antagonists, for the prevention of chemotherapy-induced nausea and vomiting (CINV). By blocking the NK1 receptors in the central nervous system, aprepitant inhibits the emetic signals triggered by various chemotherapeutic agents, thereby significantly reducing both acute and delayed phases of CINV. Its long duration of action allows for once-daily dosing, providing sustained antiemetic protection.

This mechanism of action makes it a crucial component in managing the highly emetogenic effects of certain chemotherapy regimens, improving patient comfort and adherence to treatment. Aprepitant is particularly valuable in preventing delayed CINV, which can be challenging to control with other antiemetics alone, thus enhancing the overall quality of life for patients undergoing chemotherapy.

Uses

  • Prevention of acute chemotherapy-induced nausea and vomiting.
  • Prevention of delayed chemotherapy-induced nausea and vomiting.
  • Used in combination with other antiemetics for highly emetogenic chemotherapy.
  • Management of nausea and vomiting post-surgery (off-label for oral form).

Directions For Use

Administered intravenously by a healthcare professional. Dosage and duration are determined by the treating physician based on the chemotherapy regimen.

Safety Measures

  • Alcohol - Alcohol consumption should be avoided or minimized as it may exacerbate nausea and vomiting, potentially counteracting the medication's effects.
  • Pregnancy - Use in pregnancy is generally not recommended unless clearly necessary, as animal studies have shown some potential for developmental toxicity. Consult a doctor.
  • Breastfeeding - It is unknown if aprepitant is excreted in human milk. A decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother.
  • Liver - Use with caution in patients with severe hepatic impairment, as aprepitant is extensively metabolized by the liver. Dose adjustments may be necessary.
  • Kidney - No dose adjustment is required for patients with renal impairment or end-stage renal disease.
  • Lung - Generally considered safe for use in patients with lung conditions, as it does not directly affect respiratory function.
