About

Daunorubicin is an anthracycline topoisomerase inhibitor, a potent cytotoxic antibiotic used in the treatment of various cancers. It exerts its antineoplastic effect primarily by intercalating into DNA, inhibiting topoisomerase II, and generating free radicals. These actions lead to DNA damage, inhibition of DNA and RNA synthesis, and ultimately, cell death, particularly in rapidly proliferating cells. It is a cell cycle non-specific agent, meaning it can kill cells at various stages of the cell cycle.

Administered intravenously, daunorubicin is rapidly distributed and metabolized, primarily in the liver. Its use is often limited by dose-dependent cardiotoxicity, which necessitates careful monitoring of cardiac function throughout treatment. It is a cornerstone in the treatment of acute leukemias, often used in combination with other chemotherapeutic agents to achieve remission.

Uses

  • Treatment of acute myeloid leukemia (AML).
  • Management of acute lymphoblastic leukemia (ALL).
  • Used in certain types of neuroblastoma.
  • Part of combination chemotherapy regimens.

Directions For Use

Administered intravenously by a qualified healthcare professional. Dosage and treatment schedule are determined by the physician based on the specific cancer and patient factors.

Benefits

  • Highly effective against acute leukemias.
  • Induces remission in a significant number of patients.
  • Crucial component of standard induction regimens.
  • Well-established efficacy in hematologic malignancies.
  • Can improve survival rates in acute leukemias.
  • Available for combination therapy protocols.

Side Effects

  • Cardiotoxicity (dose-dependent, cumulative)
  • Bone marrow suppression (myelosuppression)
  • Nausea and vomiting
  • Mucositis (stomatitis, esophagitis)
  • Alopecia (hair loss)
  • Red discoloration of urine
  • Diarrhea
  • Fever and chills
  • Local tissue necrosis if extravasation occurs
  • Hypersensitivity reactions
  • Liver enzyme elevation
  • Secondary malignancies (rare)

Safety Measures

  • Alcohol - Avoid alcohol as it may increase the risk of liver toxicity and exacerbate gastrointestinal side effects.
  • Pregnancy - Daunorubicin is highly teratogenic and embryotoxic. It is contraindicated in pregnancy. Women of childbearing potential must use effective contraception during and after treatment.
  • Breastfeeding - Breastfeeding is contraindicated during daunorubicin therapy and for a period after, due to the potential for serious adverse effects in the infant.
  • Liver - Use with extreme caution in patients with hepatic impairment; dose reduction is often necessary due to impaired metabolism and increased risk of toxicity, especially cardiotoxicity.
  • Kidney - Use with caution in patients with renal impairment; dose adjustments may be required as a portion of the drug is excreted renally, increasing the risk of toxicity.
  • Lung - While not a primary concern, monitor for signs of pulmonary toxicity, though it is less common than with other anthracyclines. Pre-existing lung conditions should be considered.
