About

Isosorbide mononitrate is a long-acting nitrate used for the prevention of angina pectoris (chest pain) due to coronary artery disease. It is an active metabolite of isosorbide dinitrate and works by releasing nitric oxide (NO) in vascular smooth muscle. Nitric oxide activates guanylate cyclase, leading to an increase in cyclic GMP (cGMP), which ultimately results in dephosphorylation of myosin light chains and relaxation of smooth muscle cells. This causes vasodilation of both arteries and veins, with a predominant effect on veins, leading to decreased preload (reduced venous return to the heart) and, to a lesser extent, decreased afterload.

This reduction in cardiac workload and improved coronary blood flow helps to alleviate and prevent anginal attacks. Isosorbide mononitrate is particularly useful for patients requiring sustained nitrate therapy to manage chronic stable angina, providing a consistent therapeutic effect throughout the day. It is not intended for the acute relief of angina attacks, for which rapidly acting nitrates are preferred.
