About

Metformin is an oral antidiabetic agent belonging to the biguanide class, widely recognized as a first-line treatment for type 2 diabetes mellitus. Its primary mechanism of action involves reducing hepatic glucose production, mainly by inhibiting gluconeogenesis and glycogenolysis in the liver. This significantly decreases the amount of glucose released by the liver into the bloodstream. Metformin also improves insulin sensitivity in peripheral tissues, leading to increased glucose uptake and utilization by muscle cells, and decreases intestinal absorption of glucose.

Unlike some other antidiabetic drugs, metformin does not stimulate insulin secretion, thus carrying a low risk of hypoglycemia when used as monotherapy. Beyond its glucose-lowering effects, metformin often leads to modest weight loss or weight stability and can improve lipid profiles. It is typically prescribed as an adjunct to diet and exercise, and can be used alone or in combination with other antidiabetic medications to achieve optimal glycemic control.

Uses

  • Treatment of type 2 diabetes mellitus.
  • To improve glycemic control in adults and children (10 years and older).
  • Used as monotherapy or in combination with other antidiabetic agents.
  • As an adjunct to diet and exercise.

Directions For Use

Take this medication orally, usually two to three times daily with meals, to minimize gastrointestinal side effects, or as directed by your physician. Swallow the tablet whole.

Benefits

  • Effectively lowers blood glucose levels.
  • Reduces hepatic glucose production.
  • Improves insulin sensitivity.
  • Low risk of hypoglycemia (as monotherapy).
  • Contributes to modest weight loss.
  • May improve lipid profiles.

Side Effects

  • Nausea
  • Diarrhea

Safety Measures

  • Alcohol - Avoid excessive alcohol intake, as it can significantly increase the risk of lactic acidosis, a rare but serious side effect of metformin.
  • Pregnancy - Not generally recommended during pregnancy. Insulin is typically preferred for glycemic control in pregnant women with diabetes. Consult a doctor.
  • Breastfeeding - Use with caution during breastfeeding. Metformin is excreted into breast milk, but generally considered compatible with monitoring for infant side effects.
  • Liver - Contraindicated in patients with severe liver impairment or acute liver disease due to an increased risk of lactic acidosis.
  • Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustment is required for moderate impairment.
  • Lung - Use with caution in conditions causing hypoxia (e.g., severe lung disease, acute heart failure) as these can increase the risk of lactic acidosis.
