About

Mercaptopurine is a crucial antimetabolite antineoplastic agent, classified as a purine analog, primarily indicated for the treatment of acute lymphoblastic leukemia (ALL) and certain other leukemias. Its therapeutic efficacy stems from its ability to act as a false metabolite, profoundly interfering with nucleic acid synthesis within rapidly dividing cancer cells. Specifically, mercaptopurine is converted intracellularly by hypoxanthine-guanine phosphoribosyltransferase (HGPRT) into its active metabolites, 6-thioguanine nucleotides (6-TGNs). These 6-TGNs are then incorporated into both DNA and RNA, leading to the inhibition of de novo purine synthesis and disruption of DNA replication and transcription. This ultimately results in cell cycle arrest and programmed cell death (apoptosis) in malignant cells. The metabolism of mercaptopurine is significantly influenced by the activity of the enzyme thiopurine methyltransferase (TPMT), which can necessitate genetic testing to guide dosing and minimize toxicity.
