About

Mycophenolate sodium (MPA) is an immunosuppressant indicated for the prophylaxis of organ rejection in adult patients receiving allogeneic renal, cardiac, or hepatic transplants, typically in combination with other immunosuppressants like cyclosporine and corticosteroids. It works by reversibly inhibiting inosine monophosphate dehydrogenase (IMPDH), an enzyme critical for the de novo synthesis of guanosine nucleotides. Lymphocytes are highly dependent on this pathway for proliferation, making them particularly susceptible to MPA's effects.

This selective inhibition leads to potent antiproliferative effects on T and B lymphocytes, thereby suppressing the immune response responsible for graft rejection. The enteric-coated formulation helps reduce gastrointestinal side effects, improving patient tolerability and adherence to long-term therapy, which is vital for maintaining graft function and preventing chronic rejection.

Uses

  • Prophylaxis of organ rejection in renal transplant.
  • Prophylaxis of organ rejection in cardiac transplant.
  • Prophylaxis of organ rejection in hepatic transplant.
  • Treatment of refractory rejection (off-label in some cases).

Directions For Use

Take orally twice daily on an empty stomach, as directed by your transplant physician. Do not crush or chew tablets.

Benefits

  • Effective in preventing organ rejection.
  • Selective inhibition of lymphocyte proliferation.
  • Often used in combination regimens.
  • Enteric-coated formulation reduces GI side effects.
  • Improves graft survival rates.
  • Well-established in transplant protocols.

Side Effects

  • Diarrhea
  • Nausea
  • Vomiting
  • Abdominal pain
  • Leukopenia (low white blood cell count)
  • Anemia
  • Thrombocytopenia
  • Increased risk of infections (viral, bacterial, fungal)
  • Increased risk of malignancies (e.g., lymphoma)
  • Headache
  • Hypertension
  • Tremor

Safety Measures

  • Alcohol - Alcohol should be avoided as it can exacerbate gastrointestinal side effects and potentially increase the risk of liver toxicity.
  • Pregnancy - Mycophenolate is teratogenic and embryotoxic. It is contraindicated in pregnancy. Effective contraception must be used.
  • Breastfeeding - Mycophenolate is excreted in breast milk. Breastfeeding is not recommended due to potential serious adverse effects in the infant.
  • Liver - Use with caution in patients with severe liver impairment. Close monitoring of liver function and blood counts is advised.
  • Kidney - No dosage adjustment is typically required for patients with severe chronic renal impairment. However, monitor blood counts closely.
  • Lung - Rare cases of interstitial lung disease and pulmonary fibrosis have been reported. Monitor for new or worsening respiratory symptoms.
