About

Amphotericin B is a polyene antifungal antibiotic indicated for the treatment of severe, progressive, and potentially life-threatening fungal infections, including systemic candidiasis, cryptococcosis, aspergillosis, mucormycosis, and blastomycosis. It exerts its fungicidal action by binding to ergosterol, a vital component of the fungal cell membrane, leading to the formation of pores and increased membrane permeability. This disruption causes leakage of intracellular components and ultimately fungal cell death.

Due to its broad spectrum of activity and potent fungicidal properties, amphotericin B remains a cornerstone therapy for invasive fungal infections, particularly in immunocompromised patients, despite its known toxicity profile. Newer lipid formulations have been developed to mitigate some of its severe side effects, improving its tolerability while maintaining its efficacy against serious fungal pathogens.

Uses

  • Treatment of severe systemic fungal infections.
  • Invasive candidiasis.
  • Cryptococcal meningitis.
  • Aspergillosis.
  • Mucormycosis.

Directions For Use

Administered intravenously by a healthcare professional, typically over 2-6 hours. Dosage and duration depend on the infection and patient response.

Benefits

  • Broad spectrum of antifungal activity.
  • Potent fungicidal action.
  • Effective against life-threatening infections.
  • Crucial for immunocompromised patients.
  • Established efficacy over decades.
  • Available in lipid formulations to reduce toxicity.

Side Effects

  • Fever
  • Chills
  • Nausea
  • Vomiting
  • Headache
  • Hypotension
  • Nephrotoxicity (renal impairment)
  • Hypokalemia
  • Hypomagnesemia
  • Anemia
  • Infusion-related reactions
  • Hepatotoxicity (rare)

Safety Measures

  • Alcohol - Alcohol should be avoided during amphotericin B treatment as it can exacerbate liver strain and dehydration, potentially worsening side effects.
  • Pregnancy - Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus.
  • Breastfeeding - It is unknown if amphotericin B is excreted in human milk. Breastfeeding is generally not recommended due to potential risks.
  • Liver - Use with caution in patients with pre-existing liver disease. Monitor liver function tests regularly during treatment.
  • Kidney - Nephrotoxicity is a major concern. Close monitoring of renal function (creatinine, BUN, electrolytes) is essential throughout therapy.
  • Lung - Acute pulmonary toxicity has been reported, especially with rapid infusion. Monitor for respiratory distress.
