About

Aprepitant is a highly selective neurokinin-1 (NK1) receptor antagonist primarily used for the prevention of chemotherapy-induced nausea and vomiting (CINV). It works by blocking the binding of substance P, a neuropeptide, to the NK1 receptors in the central nervous system, particularly in the area postrema, which is involved in the emetic reflex. This mechanism of action provides a distinct antiemetic effect, especially against the delayed phase of CINV, which is often poorly controlled by other antiemetics.

Aprepitant is typically administered as part of a multi-drug regimen, in combination with a corticosteroid (e.g., dexamethasone) and a 5-HT3 receptor antagonist (e.g., ondansetron), to achieve optimal control of both acute and delayed CINV. Its long half-life allows for once-daily dosing, contributing to patient convenience and compliance. It is metabolized by the cytochrome P450 3A4 (CYP3A4) enzyme system, necessitating careful consideration of potential drug interactions with other medications that are substrates, inhibitors, or inducers of this enzyme.

Uses

  • Prevention of acute chemotherapy-induced nausea and vomiting.
  • Prevention of delayed chemotherapy-induced nausea and vomiting.
  • Used in combination with other antiemetics for highly emetogenic chemotherapy.
  • Prevention of postoperative nausea and vomiting (PONV) in some cases.

Directions For Use

Take orally as directed by your doctor, usually starting before chemotherapy. Follow the specific dosing schedule provided.

  • Targets both acute and delayed CINV.
  • Improves patient comfort during chemotherapy.
  • Reduces the need for rescue antiemetics.
  • Can be used with other antiemetic agents.
  • Long duration of action allows for convenient dosing.

Side Effects

  • Fatigue
  • Hiccups
  • Constipation
  • Diarrhea
  • Headache
  • Dizziness
  • Loss of appetite
  • Abdominal pain
  • Dyspepsia
  • Weakness
  • Neutropenia
  • Increased liver enzymes

Safety Measures

  • Alcohol - No specific contraindication, but alcohol can exacerbate nausea and vomiting, so it's generally advisable to avoid it during chemotherapy.
  • Pregnancy - Aprepitant should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor.
  • Breastfeeding - It is unknown if aprepitant is excreted in human milk. A decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother.
  • Liver - Use with caution in patients with severe hepatic impairment as aprepitant is extensively metabolized by the liver. Dose adjustments may be necessary.
  • Kidney - No dosage adjustment is generally required for patients with renal impairment or end-stage renal disease.
  • Lung - No specific lung precautions are typically needed. However, general respiratory status should be monitored in chemotherapy patients.
