About

Imipramine is a tricyclic antidepressant (TCA) primarily used in the treatment of major depressive disorder and nocturnal enuresis in children. It exerts its therapeutic effects by inhibiting the reuptake of norepinephrine and serotonin in the presynaptic terminals of the central nervous system, thereby increasing the concentration of these neurotransmitters in the synaptic cleft. This action leads to an enhancement of mood and alleviation of depressive symptoms. Imipramine also possesses anticholinergic, antihistaminic, and alpha-adrenergic blocking properties, which contribute to its side effect profile. Despite the development of newer antidepressants, imipramine remains a valuable option for patients who do not respond to other treatments, though its use requires careful monitoring due to its narrow therapeutic index and potential for cardiac and anticholinergic adverse effects.

Safety Measures

  • Alcohol - Alcohol can potentiate the central nervous system depressant effects of imipramine, leading to increased sedation and impaired psychomotor function. Avoid concurrent use.
  • Pregnancy - Imipramine use during pregnancy has been associated with a small increased risk of congenital malformations, particularly cardiac defects. Use only if the potential benefit justifies the potential risk to the fetus.
  • Breastfeeding - Imipramine is excreted into breast milk. While the amount is generally low, monitor the infant for sedation, poor feeding, or irritability. Consult a healthcare professional.
  • Liver - Imipramine is extensively metabolized by the liver. Use with caution in patients with hepatic impairment; dose adjustments and monitoring of liver function are recommended.
  • Kidney - Use with caution in patients with renal impairment as metabolites are renally excreted. Dose adjustments may be necessary in severe renal dysfunction.
  • Lung - No specific direct adverse effects on lung function are typically noted. However, anticholinergic effects can exacerbate conditions like asthma or COPD in rare cases.
