About

Docetaxel is an antineoplastic agent belonging to the taxoid class, primarily used in chemotherapy. It exerts its cytotoxic effects by promoting the assembly of microtubules from tubulin dimers and inhibiting their depolymerization, leading to the stabilization of microtubules. This stabilization prevents the normal dynamic reorganization of the microtubule network, which is essential for vital cellular interphase and mitotic functions.

Consequently, docetaxel arrests cells in the G2/M phase of the cell cycle and induces apoptosis. Its mechanism of action makes it effective against rapidly dividing cancer cells, disrupting their ability to proliferate and spread. It is administered intravenously and is metabolized primarily by the cytochrome P450 3A4 isoenzyme system in the liver.

  • Treatment of breast cancer.
  • Management of non-small cell lung cancer.

Safety Measures

  • Alcohol - Alcohol consumption should be avoided or minimized during docetaxel treatment, as it may exacerbate side effects or interact with the medication's metabolism, particularly if the formulation contains alcohol.
  • Pregnancy - Docetaxel is contraindicated in pregnancy due to its genotoxic and teratogenic potential. Women of childbearing potential must use effective contraception during and for a period after treatment.
  • Breastfeeding - Breastfeeding is contraindicated during docetaxel therapy and for a period after the last dose, as the drug is likely excreted into breast milk and could cause serious adverse effects in the infant.
  • Liver - Docetaxel is extensively metabolized by the liver. Patients with hepatic impairment require dose adjustments or may be contraindicated, as increased drug exposure can lead to severe toxicity.
  • Kidney - Docetaxel is primarily metabolized by the liver, with minimal renal excretion. While severe renal impairment may not directly impact docetaxel clearance, caution is advised, and patients should be monitored for potential complications.
  • Lung - Docetaxel can cause pulmonary toxicity, including interstitial pneumonia or acute respiratory distress syndrome. Patients with pre-existing lung conditions should be closely monitored for respiratory symptoms.
