About

Doxorubicin injection is an anthracycline cytotoxic antibiotic widely used in chemotherapy. It exerts its antineoplastic effect primarily by intercalating into DNA, inhibiting topoisomerase II, and generating free radicals, leading to DNA damage and inhibition of nucleic acid synthesis. This mechanism disrupts cell division and induces apoptosis in rapidly proliferating cancer cells. It is effective against a broad spectrum of solid tumors and hematological malignancies.

Administered intravenously, doxorubicin is rapidly distributed throughout the body. Its metabolism occurs primarily in the liver, and it is excreted mainly via bile. Due to its potent cytotoxic action, careful monitoring of cardiac function, bone marrow suppression, and other potential toxicities is crucial during treatment to manage adverse effects and optimize therapeutic outcomes.

Uses

  • Treatment of various solid tumors.
  • Management of hematological malignancies.
  • Used in combination chemotherapy regimens.
  • Effective against certain lymphomas and leukemias.

Safety Measures

  • Alcohol - Avoid alcohol during doxorubicin treatment as it may exacerbate liver toxicity and gastrointestinal side effects.
  • Pregnancy - Doxorubicin is contraindicated in pregnancy due to its teratogenic and embryotoxic potential. Effective contraception is required.
  • Breastfeeding - Breastfeeding is contraindicated during doxorubicin therapy as the drug is excreted into breast milk and can harm the infant.
  • Liver - Use with extreme caution in patients with impaired liver function; dosage adjustments are often necessary due to altered metabolism and increased toxicity risk.
  • Kidney - Doxorubicin is primarily excreted via bile, but caution is advised in severe renal impairment, though significant dosage adjustment is less common than with liver issues.
  • Lung - While not a primary lung toxicant, patients with pre-existing lung conditions should be monitored, especially if receiving concomitant radiation or other lung-toxic agents.
