About

Fosaprepitant is an intravenous prodrug of aprepitant, a highly selective neurokinin-1 (NK1) receptor antagonist. Upon administration, it is rapidly converted to its active metabolite, aprepitant, in vivo. Aprepitant then exerts its antiemetic effect by blocking the binding of substance P, a key neuropeptide, to the NK1 receptor in the central nervous system, particularly in the area postrema. This action effectively prevents both the acute and, importantly, the delayed phases of chemotherapy-induced nausea and vomiting (CINV). Fosaprepitant is typically administered as a single dose prior to chemotherapy and is used in combination with other antiemetic agents, such as corticosteroids (e.g., dexamethasone) and 5-HT3 receptor antagonists (e.g., ondansetron), to provide comprehensive and enhanced control of emesis in patients receiving highly or moderately emetogenic chemotherapy, offering a convenient intravenous option.
