About

Kerendia 20mg Tablet contains Finerenone, a novel, non-steroidal, selective mineralocorticoid receptor (MR) antagonist. It is indicated for reducing the risk of sustained eGFR decline, end-stage kidney disease, cardiovascular death, non-fatal myocardial infarction, and hospitalization for heart failure in adult patients with chronic kidney disease (CKD) associated with type 2 diabetes. Finerenone works by blocking overactivation of the mineralocorticoid receptor, which is implicated in the pathogenesis of CKD and cardiovascular disease through pro-inflammatory and pro-fibrotic pathways.

Unlike traditional steroidal MR antagonists, Finerenone has a more selective binding profile and does not significantly cross the blood-brain barrier, potentially leading to a more favorable side effect profile. Its use represents a significant advancement in the management of CKD in diabetic patients, offering kidney and cardiovascular protection beyond glycemic and blood pressure control. Regular monitoring of serum potassium and renal function is crucial during treatment.

Uses

  • Reduces risk of sustained eGFR decline in CKD with type 2 diabetes.
  • Reduces risk of end-stage kidney disease in CKD with type 2 diabetes.
  • Reduces risk of cardiovascular death in CKD with type 2 diabetes.
  • Reduces risk of non-fatal MI and hospitalization for heart failure.

Directions For Use

Take one tablet orally once daily with or without food. Do not take with grapefruit or grapefruit juice.

Benefits

  • Provides kidney protection in CKD with type 2 diabetes.
  • Reduces the risk of progression to end-stage kidney disease.
  • Offers cardiovascular benefits, reducing heart failure hospitalizations.
  • Acts via a novel, non-steroidal mechanism.
  • Can be used in conjunction with other standard therapies.
  • Helps improve long-term outcomes for diabetic CKD patients.

Safety Measures

  • Alcohol - No specific interaction with alcohol is known. However, excessive alcohol can worsen underlying conditions. Moderate intake is generally acceptable.
  • Pregnancy - Finerenone is not recommended during pregnancy due to potential risks to the fetus. Use only if the potential benefit justifies the potential risk.
  • Breastfeeding - It is unknown if Finerenone is excreted in human milk. A decision should be made whether to discontinue nursing or the drug.
  • Liver - Contraindicated in patients with severe hepatic impairment. Use with caution in moderate hepatic impairment.
  • Kidney - Contraindicated in patients with severe renal impairment (eGFR <15 mL/min/1.73 m²). Regular monitoring of serum potassium and eGFR is essential.
  • Lung - Finerenone does not typically affect lung function directly and can generally be used in patients with lung conditions.
