Leuprolide is a synthetic nonapeptide analog of naturally occurring gonadotropin-releasing hormone (GnRH). Its primary mechanism of action involves initially stimulating, then subsequently desensitizing and downregulating, GnRH receptors in the anterior pituitary gland. This biphasic effect leads to a profound and sustained suppression of pituitary gonadotropin secretion, specifically luteinizing hormone (LH) and follicle-stimulating hormone (FSH). Consequently, the production of sex steroids, such as testosterone in males and estrogen in females, is significantly reduced to castrate levels. This hormonal suppression is therapeutically beneficial in managing various hormone-dependent conditions, including advanced prostate cancer, endometriosis, uterine fibroids, and central precocious puberty.
