About
Meropenem is a carbapenem antibiotic, a class of beta-lactam antibiotics with an exceptionally broad spectrum of bactericidal activity. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Meropenem is highly stable to hydrolysis by most beta-lactamases, including extended-spectrum beta-lactamases (ESBLs) and AmpC beta-lactamases, making it effective against many multidrug-resistant bacteria.
It is typically administered intravenously for serious infections, including complicated intra-abdominal infections, complicated skin and soft tissue infections, bacterial meningitis, and hospital-acquired pneumonia. Its potent activity against both gram-positive and gram-negative aerobic and anaerobic bacteria makes it a crucial agent for empirical therapy in critically ill patients, especially when resistance is a concern.
Uses
- Treatment of complicated intra-abdominal infections.
- Management of bacterial meningitis.
- Therapy for complicated skin and soft tissue infections.
- Treatment of hospital-acquired pneumonia.

Safety Measures
- Alcohol - No specific interaction with alcohol is known. However, alcohol consumption is generally discouraged during severe infections requiring meropenem.
- Pregnancy - Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus, as animal studies have shown some adverse effects.
- Breastfeeding - Excreted in very small amounts into breast milk; generally considered compatible with breastfeeding, but monitor the infant for potential gastrointestinal disturbances.
- Liver - Use with caution in patients with pre-existing liver disease, though dose adjustment is typically not required. Monitor liver function during prolonged therapy.
- Kidney - Dose adjustment is essential in patients with renal impairment to prevent drug accumulation and reduce the risk of adverse effects, including seizures.
- Lung - No specific contraindications related to pre-existing lung conditions. It is often used to treat severe lung infections like hospital-acquired pneumonia.
