About

Nilotinib is a highly potent and selective Bcr-Abl tyrosine kinase inhibitor, primarily indicated for the treatment of Philadelphia chromosome-positive chronic myeloid leukemia (Ph+ CML). This targeted therapy works by binding with high affinity to the ATP-binding site of the Bcr-Abl oncoprotein, which is a constitutively active tyrosine kinase driving the proliferation of leukemic cells. By inhibiting Bcr-Abl's enzymatic activity, nilotinib effectively blocks the downstream signaling pathways that promote cell growth and survival, ultimately leading to the induction of apoptosis in CML cells. Its specificity helps to minimize off-target effects, making it a more tolerable option for many patients. Nilotinib is particularly effective as a first-line treatment for newly diagnosed Ph+ CML in the chronic phase, and also for patients in chronic or accelerated phases who have developed resistance or intolerance to prior imatinib therapy, offering a significant advancement in managing this hematologic malignancy.

Uses

  • Newly diagnosed Philadelphia chromosome-positive chronic myeloid leukemia (Ph+ CML) in chronic phase
  • Ph+ CML in chronic or accelerated phase resistant or intolerant to prior therapy
  • Certain gastrointestinal stromal tumors (off-label)
  • Other Bcr-Abl positive leukemias

Directions For Use

Take orally twice daily on an empty stomach, at least two hours after a meal and one hour before the next meal, or as prescribed.

Benefits

  • High response rates in CML
  • Effective in imatinib-resistant cases
  • Improves progression-free survival

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption, as it may increase the risk of liver toxicity and pancreatitis associated with Nilotinib.
  • Pregnancy - Contraindicated during pregnancy due to potential for severe fetal harm. Effective contraception must be used.
  • Breastfeeding - Not recommended during breastfeeding as it is unknown if Nilotinib is excreted in human milk, but potential for serious adverse reactions in infants exists.
  • Liver - Use with caution in patients with hepatic impairment; dose adjustments may be necessary, and liver function should be monitored regularly.
  • Kidney - Use with caution in patients with renal impairment; dose adjustments may be required, and renal function should be monitored.
  • Lung - Rare cases of interstitial lung disease or pneumonitis have been reported; monitor for new or worsening respiratory symptoms.
