About

Cefixime is a third-generation cephalosporin antibiotic, known for its broad-spectrum activity against a variety of Gram-positive and Gram-negative bacteria. Its bactericidal action stems from its ability to inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This interference with peptidoglycan cross-linking leads to the disruption of the bacterial cell wall, ultimately causing cell lysis and death. Cefixime is particularly effective against common respiratory and urinary pathogens, including beta-lactamase producing strains.

It is well-absorbed orally and can be administered once daily, which enhances patient compliance. Cefixime is primarily excreted unchanged in the urine, making it a suitable option for urinary tract infections. It is commonly prescribed for acute bacterial exacerbations of chronic bronchitis, otitis media, pharyngitis, tonsillitis, and uncomplicated urinary tract infections. Careful consideration of patient allergies, particularly to penicillin, is crucial before initiating therapy.

Uses

  • Treatment of acute bacterial exacerbations of chronic bronchitis
  • Management of otitis media (middle ear infection)
  • Treatment of pharyngitis and tonsillitis
  • Management of uncomplicated urinary tract infections

Safety Measures

  • Alcohol - While there is no direct interaction, it is generally advisable to limit alcohol consumption during antibiotic treatment to avoid exacerbating gastrointestinal side effects.
  • Pregnancy - Cefixime is generally considered safe for use during pregnancy, categorized as Pregnancy Category B. However, it should only be used if clearly needed and under medical supervision.
  • Breastfeeding - Small amounts of cefixime are excreted in breast milk. While generally considered compatible with breastfeeding, monitor the infant for potential adverse effects like diarrhea or rash.
  • Liver - Cefixime is not extensively metabolized by the liver. However, use with caution in patients with severe liver impairment, and monitor for any signs of adverse effects.
  • Kidney - Dose adjustments are necessary in patients with significant renal impairment, as cefixime is primarily excreted by the kidneys. Close monitoring of kidney function is crucial to prevent drug accumulation.
  • Lung - Patients with a history of severe allergic reactions, especially to penicillin, should be monitored for cross-reactivity, which could manifest as respiratory distress or bronchospasm.
