PHYTORAL TAB, containing Ketoconazole, is an imidazole antifungal agent utilized for the treatment of various fungal infections. Its primary mechanism of action involves the inhibition of 14-alpha-demethylase, a cytochrome P450 enzyme crucial for the synthesis of ergosterol, a vital component of fungal cell membranes. This disruption leads to increased permeability of the fungal cell membrane, leakage of essential cellular contents, and ultimately, fungal cell death. Ketoconazole exhibits a broad spectrum of activity against various fungi, including dermatophytes, yeasts (such as Candida species), and other pathogenic fungi. While it was historically employed for systemic fungal infections, its systemic use is now more restricted due to the potential for significant liver toxicity and numerous drug interactions. It is often reserved for specific conditions where other antifungals are not tolerated or effective, or for its off-label use in conditions like Cushing's syndrome due to its ability to inhibit steroid synthesis.
