About

TRIOLMEZEST 40mg is a sophisticated triple-combination antihypertensive medication designed for the comprehensive management of essential hypertension, especially in patients whose blood pressure is not adequately controlled by monotherapy or dual therapy. This formulation integrates three distinct pharmacological agents, each targeting a different mechanism involved in blood pressure regulation. Olmesartan, an angiotensin receptor blocker (ARB), works by selectively blocking the binding of angiotensin II to its AT1 receptors, leading to vasodilation and reduced aldosterone secretion, thereby lowering blood pressure.

Amlodipine, a dihydropyridine calcium channel blocker (CCB), relaxes the smooth muscles in the arterial walls by inhibiting the influx of calcium ions, resulting in peripheral vasodilation and a decrease in systemic vascular resistance. Hydrochlorothiazide, a thiazide diuretic, acts on the renal tubules to increase the excretion of sodium and water, which reduces blood volume and consequently lowers blood pressure. The synergistic action of these three components provides robust and sustained blood pressure control, effectively reducing the risk of associated cardiovascular events such as stroke and myocardial infarction.

Uses

  • Treatment of essential hypertension.
  • Blood pressure control in patients unresponsive to dual therapy.
  • Reduction of cardiovascular risk associated with high blood pressure.
  • Management of severe or resistant hypertension.

Directions For Use

Take one tablet orally once daily, preferably at the same time each day, with or without food.

Benefits

  • Provides comprehensive blood pressure control.
  • Reduces the risk of stroke and heart attack.
  • Offers synergistic antihypertensive effects.
  • Effective for resistant hypertension.
  • Convenient once-daily dosing.
  • Improves overall cardiovascular outcomes.

Side Effects

  • Dizziness
  • Headache
  • Fatigue
  • Peripheral edema (swelling)
  • Nausea
  • Hypotension (low blood pressure)
  • Cough
  • Muscle cramps
  • Increased urination
  • Electrolyte imbalances (e.g., hypokalemia)

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption as it can further lower blood pressure and increase the risk of dizziness or fainting.
  • Pregnancy - Contraindicated during the second and third trimesters due to risk of fetal harm (renal dysfunction, oligohydramnios, fetal death). Avoid in first trimester.
  • Breastfeeding - Not recommended. Components may pass into breast milk and could potentially harm the infant. Consult a doctor.
  • Liver - Use with caution in patients with mild to moderate hepatic impairment. Contraindicated in severe liver disease due to risk of hepatic coma with hydrochlorothiazide.
  • Kidney - Contraindicated in severe renal impairment (creatinine clearance <30 mL/min) and anuria. Monitor kidney function closely in other cases.
  • Lung - Use with caution in patients with a history of asthma or allergies, as thiazide diuretics can rarely exacerbate these conditions.
