About

Isosorbide mononitrate is a long-acting nitrate primarily used for the prophylaxis of angina pectoris (chest pain) due to coronary artery disease. It works by releasing nitric oxide, which activates guanylate cyclase, leading to an increase in cyclic GMP in vascular smooth muscle. This results in relaxation of both arterial and venous smooth muscle. The predominant effect is venodilation, which reduces preload on the heart by decreasing venous return. Arterial dilation also occurs, reducing systemic vascular resistance and afterload. By reducing both preload and afterload, isosorbide mononitrate decreases myocardial oxygen demand, thereby preventing anginal attacks. It is not intended for the immediate relief of acute anginal attacks but rather for sustained prophylactic treatment to reduce the frequency and severity of angina.
