About
Glyceryl Trinitrate (GTN) and Isosorbide Dinitrate are organic nitrates, a class of medications widely recognized for their potent vasodilatory properties. Their mechanism of action involves the release of nitric oxide (NO) within vascular smooth muscle cells. Nitric oxide then activates guanylate cyclase, leading to an increase in cyclic guanosine monophosphate (cGMP), which ultimately results in the relaxation of smooth muscle. This relaxation primarily affects the venous system, causing venodilation that reduces venous return to the heart (preload). A decrease in preload subsequently lowers myocardial oxygen demand, which is crucial in managing angina pectoris. Additionally, these nitrates also dilate coronary arteries, improving blood flow to ischemic areas of the heart muscle and relieving coronary vasospasm. The combined effect of reduced cardiac workload and enhanced myocardial perfusion effectively alleviates and prevents episodes of angina, improving the quality of life for patients with ischemic heart disease.
Uses
- Treatment of acute angina pectoris attacks
- Prophylaxis of angina pectoris (chest pain)
- Management of chronic stable angina
- Adjunctive therapy in heart failure (isosorbide dinitrate)
Directions For Use
Take as prescribed, usually orally. For sublingual GTN, place under the tongue at the onset of chest pain and allow to dissolve.
Benefits
- Rapidly relieves angina chest pain
- Prevents recurrent angina attacks
- Improves exercise tolerance in angina patients
- Reduces cardiac workload
- Enhances blood flow to the heart muscle
- Available in various formulations for different needs
Side Effects
- Headache (often severe)
- Dizziness or lightheadedness
- Flushing of the face and neck
- Hypotension (low blood pressure)
- Tachycardia (fast heart rate)
- Nausea or vomiting
- Weakness
- Postural hypotension (dizziness upon standing)
- Palpitations
- Syncope (fainting)
- Tolerance development with prolonged use
- Rash or skin irritation (with transdermal forms)
Safety Measures
- Alcohol - Avoid or limit alcohol as it can significantly enhance the hypotensive effects, leading to severe dizziness or fainting.
- Pregnancy - Use only if clearly needed and the potential benefits outweigh the risks. Consult a doctor. Limited data available.
- Breastfeeding - Exercise caution. It is unknown if nitrates are excreted in human milk. Consult a healthcare professional.
- Liver - Use with caution in patients with severe liver impairment as metabolism may be altered. Dose adjustment may be necessary.
- Kidney - Use with caution in patients with severe kidney impairment as excretion may be affected. Dose adjustment may be necessary.
- Lung - Generally safe for lung conditions, but use cautiously in patients with severe pulmonary hypertension or conditions that might be exacerbated by systemic hypotension.
