About

Cytarabine, commonly known as Aracyt, is an antimetabolite chemotherapeutic agent primarily used in the treatment of various hematological malignancies. It functions by interfering with DNA synthesis, specifically by inhibiting DNA polymerase, which is crucial for cell replication. This action leads to the disruption of rapidly dividing cancer cells, making it effective against leukemias and lymphomas. Its mechanism involves conversion to an active triphosphate form (ara-CTP) within the cell, which then competes with deoxycytidine triphosphate for incorporation into DNA and RNA, ultimately halting cell growth and inducing apoptosis.

This cytotoxic effect is particularly pronounced in the S-phase of the cell cycle, making it a cell-cycle specific agent. Due to its potent myelosuppressive effects, administration requires careful monitoring of blood counts and supportive care. It is typically given intravenously, subcutaneously, or intrathecally, depending on the specific indication and treatment protocol. The dosage and regimen are highly individualized based on the patient's condition, type of cancer, and response to therapy.

Uses

  • Treatment of acute myeloid leukemia (AML).
  • Management of acute lymphoblastic leukemia (ALL).
  • Therapy for chronic myeloid leukemia (CML) in blast crisis.
  • Treatment of non-Hodgkin's lymphoma.

Directions For Use

Administered intravenously, subcutaneously, or intrathecally by a healthcare professional. Dosage and frequency are determined by the treating physician.

Benefits

  • Effective in inducing remission in acute leukemias.
  • Crucial component of many chemotherapy regimens.
  • Can be used for both induction and consolidation therapy.
  • Available in various formulations for different routes of administration.
  • Helps prolong survival in certain hematological cancers.
  • Established efficacy in its indicated uses.

Side Effects

  • Bone marrow suppression (anemia, leukopenia, thrombocytopenia)
  • Nausea and vomiting
  • Diarrhea
  • Oral mucositis/stomatitis
  • Fever
  • Rash
  • Conjunctivitis (cytarabine syndrome)
  • Neurotoxicity (especially with high doses or intrathecal use)
  • Liver dysfunction
  • Hair loss (alopecia)
  • Anorexia
  • Fatigue

Safety Measures

  • Alcohol - Avoid alcohol consumption as it may exacerbate liver toxicity and gastrointestinal side effects associated with chemotherapy.
  • Pregnancy - Cytarabine is highly teratogenic and contraindicated in pregnancy due to severe fetal harm. Effective contraception is essential during treatment.
  • Breastfeeding - Breastfeeding is contraindicated during cytarabine therapy as the drug can pass into breast milk and harm the infant.
  • Liver - Use with caution in patients with pre-existing liver impairment; dose adjustments may be necessary, and liver function should be closely monitored.
  • Kidney - Use with caution in patients with renal dysfunction; dose adjustments may be required, and kidney function should be regularly assessed.
  • Lung - Rarely, cytarabine can cause pulmonary toxicity, including non-cardiogenic pulmonary edema. Monitor for respiratory symptoms.
