About

Mycophenolate mofetil (MMF) is a potent immunosuppressant agent widely used in transplant medicine to prevent the rejection of allogeneic organs, including kidney, heart, and liver transplants. Its mechanism of action involves the reversible inhibition of inosine monophosphate dehydrogenase (IMPDH), a key enzyme in the de novo pathway of guanosine nucleotide synthesis. Lymphocytes, particularly T and B cells, are highly dependent on this de novo pathway for their proliferation and function, whereas other cell types can utilize a salvage pathway. By selectively inhibiting IMPDH, MMF effectively suppresses the proliferation of T and B lymphocytes, thereby reducing the immune response that would otherwise target and reject the transplanted organ. MMF is typically administered in combination with other immunosuppressive agents, such as corticosteroids and calcineurin inhibitors, to achieve optimal immunosuppression and minimize the risk of acute and chronic rejection episodes, ultimately improving graft survival and patient outcomes post-transplantation.
