About

This medication is an antimetabolite antineoplastic agent, specifically a pyrimidine analog, primarily used in the treatment of various leukemias and lymphomas. Its mechanism of action involves intracellular conversion to an active triphosphate form (cytarabine triphosphate), which then inhibits DNA polymerase, thereby disrupting DNA synthesis and repair. It also incorporates into DNA and RNA, leading to further cellular damage and inhibition of cell proliferation. This selective toxicity against rapidly dividing cells makes it highly effective against malignant hematopoietic cells. However, its impact on healthy rapidly proliferating cells, particularly in the bone marrow and gastrointestinal tract, leads to significant side effects. It is typically administered intravenously or subcutaneously, often in high doses, as part of induction or consolidation therapy, with careful monitoring of hematological parameters and neurological status.

Uses

  • Treatment of acute myeloid leukemia (AML).
  • Management of acute lymphoblastic leukemia (ALL).
  • Therapy for chronic myeloid leukemia (CML) in blast crisis.
  • Treatment of non-Hodgkin lymphoma.

Directions For Use

Administered intravenously or subcutaneously by a healthcare professional. Dosage and schedule are highly specific to the type of leukemia or lymphoma being treated and patient tolerance.

Benefits

  • Induces remission in acute leukemias.
  • Effective in high-dose regimens for refractory disease.
  • Crucial component of many leukemia treatment protocols.
  • Can be used for CNS prophylaxis/treatment (intrathecal).
  • Improves long-term survival in specific leukemias.
  • Well-established efficacy in combination chemotherapy.

Side Effects

  • Severe myelosuppression (pancytopenia)
  • Nausea and vomiting
  • Diarrhea
  • Oral mucositis
  • Conjunctivitis (red eyes, often requiring eye drops)
  • Fever
  • Skin rash
  • Cytarabine syndrome (fever, myalgia, rash)
  • Cerebellar toxicity (ataxia, dysarthria) - especially with high doses
  • Liver dysfunction
  • Peripheral neuropathy
  • Pulmonary edema (rare, high dose)

Safety Measures

  • Alcohol - Avoid or limit alcohol consumption as it may exacerbate liver toxicity or gastrointestinal side effects associated with chemotherapy.
  • Pregnancy - Highly contraindicated during pregnancy due to severe risk of fetal harm, including birth defects and miscarriage. Effective contraception is essential.
  • Breastfeeding - Not recommended during breastfeeding as the drug can pass into breast milk and harm the infant. Discontinue breastfeeding or the medication.
  • Liver - Use with extreme caution in patients with pre-existing liver impairment; dose adjustments may be necessary, and liver function should be closely monitored.
  • Kidney - Administer cautiously in patients with renal dysfunction; dose modifications are often required, and kidney function should be regularly assessed.
  • Lung - Use with caution in patients with pre-existing lung conditions, as some chemotherapy agents can cause pulmonary toxicity. Monitor for respiratory symptoms.
