About

Aprepitant is a highly selective neurokinin-1 (NK1) receptor antagonist, a novel class of antiemetic agents. Its mechanism of action involves blocking the binding of substance P, a key neuropeptide, to the NK1 receptor in the central nervous system, particularly in the area postrema. This blockade effectively inhibits both the acute and, crucially, the delayed phases of chemotherapy-induced nausea and vomiting (CINV), which are often difficult to control. Aprepitant is typically used as part of a multi-drug regimen, in combination with other antiemetic agents such as corticosteroids (e.g., dexamethasone) and 5-HT3 receptor antagonists (e.g., ondansetron), to provide comprehensive and enhanced control of emesis in patients receiving highly or moderately emetogenic chemotherapy. Its addition significantly improves the overall efficacy of antiemetic prophylaxis.
