Gluvilda OD contains Vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, indicated for the treatment of type 2 diabetes mellitus. Vildagliptin works by selectively inhibiting the DPP-4 enzyme, which is responsible for the rapid degradation of incretin hormones, GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). By preventing their breakdown, Vildagliptin increases the circulating levels of these active incretins, leading to enhanced glucose-dependent insulin secretion from pancreatic beta cells and reduced glucagon secretion from pancreatic alpha cells. This dual action effectively lowers blood glucose concentrations, both fasting and post-prandial.

This medication is typically used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes. It can be prescribed as monotherapy for patients who cannot tolerate Metformin or for whom Metformin is contraindicated, or in combination with other oral antidiabetic agents, including Metformin, sulfonylureas, or thiazolidinediones.
