About

Leucovorin, also known as folinic acid, is a reduced form of folic acid that is metabolically active and does not require reduction by dihydrofolate reductase. This characteristic is crucial in its primary clinical application as a "rescue" agent to mitigate the severe toxic effects of methotrexate, a potent antifolate chemotherapy drug. By providing an exogenous source of reduced folate, leucovorin bypasses the methotrexate-induced inhibition of dihydrofolate reductase, allowing healthy cells to synthesize purines and pyrimidines, thereby protecting them from methotrexate's cytotoxic effects. Additionally, leucovorin is utilized to enhance the cytotoxic activity of fluorouracil in the treatment of certain cancers, particularly colorectal cancer. It achieves this by stabilizing the binding of fluorouracil's active metabolite to thymidylate synthase, thereby prolonging and intensifying the inhibition of DNA synthesis in cancer cells.
