About

Milicor Injection, containing Milrinone, is an inotropic and vasodilator agent primarily used in the short-term treatment of acute decompensated heart failure. It works by inhibiting phosphodiesterase III (PDE3) in the heart and vascular smooth muscle. This inhibition leads to an increase in intracellular cyclic AMP (cAMP), which in turn enhances myocardial contractility (positive inotropy) and promotes vasodilation (relaxation of blood vessels). The combined effect is an improvement in the heart's pumping ability and a reduction in the resistance against which the heart has to pump, leading to increased cardiac output and reduced pulmonary capillary wedge pressure.

Administered intravenously, Milicor Injection is typically used in a hospital setting for patients experiencing severe heart failure symptoms that are not adequately managed by conventional therapies. Its use requires continuous hemodynamic monitoring due to its potent effects on the cardiovascular system. The goal of therapy is to improve cardiac function, alleviate symptoms such as shortness of breath and fatigue, and stabilize the patient's condition, often as a bridge to other long-term treatments or recovery.

Uses

  • Short-term treatment of acute decompensated heart failure.
  • Improvement of cardiac output in severe heart failure.
  • Reduction of pulmonary capillary wedge pressure.
  • Management of heart failure unresponsive to other therapies.

Directions For Use

Administered intravenously by a healthcare professional in a hospital setting. Dosage is carefully titrated based on patient's hemodynamic response.

Benefits

  • Significantly improves heart's pumping function.
  • Reduces symptoms of acute heart failure.
  • Increases cardiac output and reduces systemic vascular resistance.
  • Provides rapid hemodynamic improvement.
  • Useful in critical care for severe cardiac dysfunction.
  • Can improve oxygen delivery to tissues.

Side Effects

  • Ventricular arrhythmias (e.g., PVCs, ventricular tachycardia).
  • Hypotension (low blood pressure).
  • Headache.
  • Angina or chest pain.
  • Nausea.
  • Vomiting.
  • Tremor.
  • Thrombocytopenia (low platelet count).
  • Hypokalemia (low potassium levels).
  • Injection site reactions.
  • Bronchospasm (rare).
  • Liver enzyme elevation.

Safety Measures

  • Alcohol - Not applicable in acute critical care settings where this medication is used.
  • Pregnancy - Use only if clearly indicated in life-threatening situations, as potential risks to the fetus are unknown.
  • Breastfeeding - Not recommended; it is unknown if Milrinone is excreted in human milk.
  • Liver - Use with caution in patients with liver impairment; close monitoring of liver function is advised.
  • Kidney - Dosage adjustments are required in patients with renal impairment; close monitoring of kidney function is essential.
  • Lung - Use with caution in patients with severe obstructive lung disease, as it can potentially worsen pulmonary hypertension.
