About

Isosorbide mononitrate is an organic nitrate vasodilator primarily used for the prevention of angina pectoris. Unlike dinitrate, it is the active 5-mononitrate metabolite of isosorbide dinitrate, meaning it does not undergo significant first-pass metabolism and has high bioavailability. It works by releasing nitric oxide, which relaxes vascular smooth muscle, leading to vasodilation. This effect is more pronounced on veins, reducing venous return to the heart (preload) and consequently decreasing myocardial oxygen demand. It also causes some arterial dilation, which can improve coronary blood flow. This standard-release formulation typically requires multiple daily doses to maintain therapeutic levels and prevent angina attacks. It is not intended for the immediate relief of acute angina.

Uses

  • Prophylaxis of angina pectoris (chest pain).
  • Long-term management of coronary artery disease.
  • Adjunctive therapy in chronic heart failure.
  • Reduction of cardiac workload in certain cardiovascular conditions.
