About

Isosorbide dinitrate is an organic nitrate vasodilator used for the prevention and treatment of angina pectoris. Similar to other nitrates, it works by being metabolized to nitric oxide, which activates guanylate cyclase and increases cyclic GMP in vascular smooth muscle. This leads to relaxation of both arterial and venous smooth muscle, with a predominant effect on venodilation. The reduction in venous return (preload) decreases the workload on the heart, while arterial dilation reduces afterload, both contributing to decreased myocardial oxygen demand. It also directly dilates coronary arteries, improving blood flow to ischemic areas. Isosorbide dinitrate has a relatively rapid onset of action compared to mononitrate, but a shorter duration, making it suitable for both acute angina relief and prophylactic use, often in multiple daily doses.

Uses

  • Prevention of angina pectoris.
  • Treatment of acute angina attacks (sublingual forms).
  • Adjunctive therapy in congestive heart failure.
  • Management of pulmonary hypertension.

  • Breastfeeding - It is unknown if isosorbide dinitrate passes into breast milk. Exercise caution and consult a healthcare professional.
  • Liver - Use with caution in patients with severe liver impairment, as metabolism may be altered. Dose adjustment might be necessary.
  • Kidney - Generally considered safe in kidney disease, but caution is advised in severe renal impairment. No specific dose adjustment usually required.
  • Lung - No specific contraindications related to lung conditions, but use with caution in patients with severe pulmonary hypertension.
