About

Amphotericin B Liposomal is an antifungal agent used to treat severe, life-threatening systemic fungal infections. It is a lipid formulation of amphotericin B, a polyene macrolide antibiotic. The liposomal encapsulation delivers amphotericin B directly to fungal cells while minimizing its exposure to human cells, thereby significantly reducing the drug's notorious nephrotoxicity and infusion-related reactions compared to conventional amphotericin B. The drug works by binding to ergosterol, a sterol unique to fungal cell membranes, leading to the formation of pores and increased membrane permeability. This disruption causes leakage of intracellular components and ultimately fungal cell death. Its broad spectrum of activity against various fungi makes it a crucial treatment option for immunocompromised patients.

Uses

  • Invasive aspergillosis
  • Systemic candidiasis
  • Cryptococcosis
  • Empiric therapy for febrile neutropenia

Directions For Use

Administer intravenously as an infusion over 2 hours, as directed by a healthcare professional. Do not mix with other medications.

Benefits

  • Effective against a broad range of systemic fungal infections.
  • Significantly reduced nephrotoxicity compared to conventional Amphotericin B.
  • Lower incidence of infusion-related reactions.
  • Improved tolerability for patients.
  • Crucial for immunocompromised patients.
  • Enhanced drug delivery to fungal cells.

Side Effects

  • Fever
  • Chills
  • Nausea
  • Vomiting
  • Headache
  • Hypokalemia (low potassium)
  • Hypomagnesemia (low magnesium)
  • Anemia
  • Increased creatinine (less than conventional form)
  • Infusion-related reactions
  • Rash
  • Abdominal pain

Safety Measures

  • Alcohol - No specific interaction, but alcohol may exacerbate general discomfort or liver strain.
  • Pregnancy - Use only if clearly needed; potential benefits must outweigh potential risks to the fetus.
  • Breastfeeding - Not recommended; it is unknown if amphotericin B is excreted in human milk.
  • Liver - Use with caution in patients with pre-existing liver impairment; monitor liver function tests.
  • Kidney - While less nephrotoxic than conventional Amphotericin B, monitor renal function closely, especially in pre-existing kidney disease.
  • Lung - Monitor for acute pulmonary toxicity, including dyspnea and bronchospasm, especially during infusion.
