About

Tenofovir Alafenamide (TAF) is a novel prodrug of tenofovir, a nucleotide reverse transcriptase inhibitor (NRTI) used in the treatment of HIV-1 infection and chronic hepatitis B virus (HBV) infection. TAF is designed to deliver tenofovir more efficiently to target cells, particularly lymphocytes and hepatocytes, at a lower dose compared to its predecessor, tenofovir disoproxil fumarate (TDF). This improved intracellular delivery results in higher intracellular concentrations of the active metabolite, tenofovir diphosphate, with significantly lower plasma concentrations of tenofovir. The advantage of TAF lies in its reduced systemic exposure to tenofovir, which translates to improved renal and bone safety profiles compared to TDF, making it a preferred option for many patients requiring long-term antiviral therapy.

Uses

  • Treatment of HIV-1 infection
  • Treatment of chronic Hepatitis B virus (HBV) infection
  • Part of combination antiretroviral therapy
  • Prevention of HIV (PrEP, though TDF is more common)

Directions For Use

Take orally once daily with food to enhance absorption. Adhere strictly to the prescribed regimen.

Benefits

  • Highly effective against HIV-1 and HBV.
  • Improved renal safety profile compared to TDF.
  • Improved bone safety profile compared to TDF.
  • Lower dose required for efficacy.
  • Part of convenient once-daily regimens.
  • Reduced systemic tenofovir exposure.

Side Effects

  • Nausea
  • Diarrhea
  • Headache
  • Fatigue
  • Abdominal pain
  • Flatulence
  • Dizziness
  • Rash
  • Increased liver enzymes
  • Lactic acidosis (rare)
  • Renal impairment (less common than TDF)
  • Bone mineral density decrease (less common than TDF)

Safety Measures

  • Alcohol - Limit alcohol intake, especially in HBV patients, to avoid exacerbating liver disease.
  • Pregnancy - Generally considered safe; use if clinically indicated, with careful monitoring.
  • Breastfeeding - Not recommended for HIV-positive mothers due to risk of HIV transmission and potential drug effects.
  • Liver - Use with caution in decompensated liver disease; monitor liver function closely.
  • Kidney - Generally safer than TDF, but monitor renal function, especially in pre-existing impairment.
  • Lung - No specific lung-related safety concerns; generally well-tolerated in patients with lung conditions.
