About

Sevelamer is a non-calcium, non-aluminum phosphate binder used to control serum phosphorus levels in patients with chronic kidney disease (CKD) on dialysis. In CKD, impaired kidney function leads to elevated phosphorus levels, which can cause bone disease, cardiovascular calcification, and other complications. Sevelamer works by binding to dietary phosphate in the gastrointestinal tract, forming an insoluble complex that is then excreted in the feces, thereby preventing its absorption into the bloodstream. It does not contain calcium or aluminum, which helps avoid the risks associated with calcium overload or aluminum toxicity. Regular monitoring of serum phosphorus, calcium, and parathyroid hormone levels is essential during treatment to optimize patient outcomes and prevent long-term complications. This therapeutic approach is crucial for managing the complex metabolic disturbances seen in advanced renal failure.

Uses

  • Control of serum phosphorus in patients with CKD on dialysis.
  • Reduction of hyperphosphatemia in non-dialysis CKD patients (off-label).
  • Prevention of cardiovascular calcification associated with hyperphosphatemia.
  • Management of secondary hyperparathyroidism by lowering phosphorus.

Directions For Use

Take orally with meals. Do not crush, chew, or break tablets. Follow the prescribed dosage strictly.

Benefits

  • Effectively lowers serum phosphorus levels.
  • Non-calcium, non-aluminum formulation.
  • Reduces risk of calcium overload and aluminum toxicity.
  • Helps prevent progression of bone disease in CKD.
  • May reduce cardiovascular calcification.
  • Improves overall management of chronic kidney disease.

Side Effects

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain
  • Indigestion
  • Flatulence
  • Dyspepsia
  • Bowel obstruction (rare)
  • Peritonitis (in peritoneal dialysis patients)
  • Pruritus
  • Headache

Safety Measures

  • Alcohol - No specific interaction, but general healthy lifestyle choices are important for CKD patients.
  • Pregnancy - Use only if clearly needed; animal studies show no direct harm, but human data are limited. Consult a physician.
  • Breastfeeding - Unlikely to be absorbed systemically, so generally considered safe, but consult a healthcare professional.
  • Liver - No dosage adjustment needed for liver conditions as it is not systemically absorbed.
  • Kidney - Specifically indicated for patients with chronic kidney disease, particularly those on dialysis.
  • Lung - No specific concerns for lung conditions as it acts locally in the GI tract.
