About

Sevincio-400, containing sevelamer, is a non-calcium, non-aluminum phosphate binder used to control serum phosphorus levels in patients with chronic kidney disease (CKD) on dialysis. In CKD, kidneys are unable to excrete phosphate effectively, leading to hyperphosphatemia, which can cause secondary hyperparathyroidism, renal osteodystrophy, and cardiovascular calcification. Sevelamer works by binding to dietary phosphate in the gastrointestinal tract, forming an insoluble complex that is then excreted in the feces. This reduces the absorption of phosphate into the bloodstream, thereby lowering serum phosphate levels.

Unlike calcium-based binders, sevelamer does not contribute to calcium load, which is beneficial for patients at risk of vascular calcification. It also has a beneficial effect on lipid profiles, reducing total and LDL cholesterol. Sevelamer is not absorbed systemically, making its action localized to the gut. Effective management of hyperphosphatemia is crucial for improving patient outcomes and reducing the complications associated with CKD.

Uses

  • Control of serum phosphorus in patients with chronic kidney disease on dialysis.
  • Prevention of hyperphosphatemia-related complications.
  • Management of secondary hyperparathyroidism in CKD.
  • Reduction of total and LDL cholesterol levels.

Directions For Use

Take tablets orally with meals, as directed by your physician. Swallow whole; do not crush, chew, or break.

Benefits

  • Effectively lowers serum phosphate levels.
  • Non-calcium, non-aluminum based binder.
  • Reduces risk of vascular calcification.
  • Improves lipid profiles (lowers cholesterol).
  • Not systemically absorbed.
  • Helps manage complications of CKD.

Side Effects

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain
  • Dyspepsia
  • Flatulence
  • Bowel obstruction (rare)
  • Peritonitis (in peritoneal dialysis patients)
  • Dysphagia
  • Pruritus
  • Hypophosphatemia (if dose is too high)

Safety Measures

  • Alcohol - No specific interaction with alcohol; however, maintaining overall health is important for CKD patients.
  • Pregnancy - Use with caution; consult a doctor. Limited data available, but systemic absorption is minimal.
  • Breastfeeding - Use with caution; consult a doctor. Systemic absorption is minimal, so unlikely to pass into breast milk.
  • Liver - No specific dose adjustment needed as sevelamer is not metabolized by the liver and is not systemically absorbed.
  • Kidney - Specifically indicated for patients with chronic kidney disease on dialysis; no dose adjustment needed for kidney function itself, but dosage is tailored to phosphate levels.
  • Lung - No specific considerations for lung conditions.
