About

Telmisartan is an angiotensin II receptor blocker (ARB) that selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. Angiotensin II is a potent vasoconstrictor and a primary hormone of the renin-angiotensin-aldosterone system (RAAS), which plays a crucial role in the pathophysiology of hypertension and heart failure. By blocking its effects, telmisartan causes vasodilation, reduces aldosterone secretion, and lowers blood pressure. It is primarily indicated for the treatment of essential hypertension and for cardiovascular risk reduction in patients aged 55 years or older at high risk of cardiovascular events who are unable to take ACE inhibitors. Its long duration of action allows for once-daily dosing, providing consistent blood pressure control.

Uses

  • Treatment of essential hypertension.
  • Reduction of cardiovascular morbidity and mortality.
  • Prevention of stroke and myocardial infarction in high-risk patients.
  • Management of diabetic nephropathy (off-label, but common for ARBs).

Directions For Use

Take one tablet orally once daily, with or without food, as directed by your physician. It is best taken at the same time each day.

Benefits

  • Effective and sustained blood pressure reduction.
  • Reduced risk of cardiovascular events.
  • Provides 24-hour blood pressure control.
  • Well-tolerated with a low incidence of cough.
  • Can be used in patients intolerant to ACE inhibitors.
  • Protective effects on target organs like kidneys and heart.

Side Effects

  • Dizziness.
  • Headache.
  • Fatigue.
  • Nausea.
  • Diarrhea.
  • Back pain.
  • Upper respiratory tract infection.
  • Hypotension (low blood pressure).
  • Hyperkalemia (elevated potassium levels).
  • Angioedema (rare).
  • Renal impairment (especially in susceptible patients).
  • Cough (less common than ACE inhibitors).

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can further lower blood pressure and increase the risk of dizziness.
  • Pregnancy - Contraindicated during the second and third trimesters of pregnancy due to potential fetal toxicity. Not recommended in the first trimester.
  • Breastfeeding - Not recommended during breastfeeding as telmisartan may pass into breast milk and could harm the infant.
  • Liver - Use with caution in patients with mild to moderate liver impairment. Contraindicated in severe liver impairment or biliary obstruction.
  • Kidney - Use with caution in patients with renal artery stenosis or severe kidney impairment. Monitor kidney function and potassium levels regularly.
  • Lung - Generally safe for use in patients with lung conditions. Less likely to cause cough compared to ACE inhibitors.
