About

Cefoperazone/Sulbactam is a combination antibiotic consisting of a third-generation cephalosporin, cefoperazone, and a beta-lactamase inhibitor, sulbactam. Cefoperazone exerts its bactericidal action by inhibiting bacterial cell wall synthesis, primarily targeting penicillin-binding proteins. Sulbactam irreversibly inhibits a wide range of beta-lactamases, enzymes produced by bacteria that can inactivate cefoperazone. This combination broadens the spectrum of activity of cefoperazone to include many beta-lactamase-producing bacteria, making it effective against a wide range of Gram-positive and Gram-negative aerobic and anaerobic pathogens, particularly useful for serious infections.

The synergistic action of cefoperazone and sulbactam provides enhanced antimicrobial coverage, especially against resistant strains that produce beta-lactamases. This makes it a valuable option for treating complicated infections where resistance is a concern. Its broad spectrum and efficacy against various difficult-to-treat pathogens, including those causing nosocomial infections, underscore its importance in clinical practice. It is typically administered parenterally due to its formulation.

Uses

  • Treatment of respiratory tract infections.
  • Management of urinary tract infections.
  • Treatment of intra-abdominal infections.
  • Management of skin and soft tissue infections.

Directions For Use

Administered intravenously or intramuscularly by a healthcare professional. Dosage and duration depend on the infection severity and patient's condition.

Benefits

  • Broad spectrum of antibacterial activity.
  • Effective against beta-lactamase producing bacteria.
  • Useful for severe and complicated infections.
  • Covers both Gram-positive and Gram-negative pathogens.
  • Effective against anaerobic bacteria.
  • Good tissue penetration.

Side Effects

  • Diarrhea
  • Nausea
  • Vomiting
  • Rash
  • Pruritus
  • Fever
  • Injection site pain
  • Phlebitis
  • Eosinophilia
  • Transient elevations in liver enzymes
  • Hypoprothrombinemia (due to cefoperazone)
  • Pseudomembranous colitis (rare)

Safety Measures

  • Alcohol - Avoid alcohol consumption during and for several days after treatment due to the disulfiram-like reaction risk with cefoperazone.
  • Pregnancy - Use during pregnancy only if clearly needed and potential benefits outweigh risks. Consult a doctor.
  • Breastfeeding - Both components are excreted in breast milk. Use with caution; monitor infant for gastrointestinal disturbances.
  • Liver - Use with caution in patients with hepatic impairment or biliary obstruction. Dose adjustment may be necessary.
  • Kidney - Dose adjustment is generally not required for renal impairment alone, but combined hepatic and renal impairment needs careful consideration.
  • Lung - Generally safe for use in patients with lung conditions, as it does not directly affect respiratory function.
