About

Ticagrelor (Brilinta) is an oral antiplatelet medication belonging to the class of P2Y12 platelet inhibitors. It works by reversibly binding to the P2Y12 receptor on platelets, preventing adenosine diphosphate (ADP) from binding and activating the receptor. This action inhibits platelet aggregation, a crucial step in the formation of blood clots. Unlike clopidogrel and prasugrel, ticagrelor is a direct-acting drug that does not require metabolic activation, leading to a faster onset of action and more consistent antiplatelet effect.

It is primarily used in patients with acute coronary syndromes (ACS) or a history of myocardial infarction (MI) to reduce the risk of thrombotic cardiovascular events. Its efficacy has been demonstrated in large clinical trials, showing superiority over clopidogrel in certain high-risk populations, though it carries an increased risk of bleeding.

Uses

  • Prevention of thrombotic events in patients with acute coronary syndrome (ACS).
  • Reduction of cardiovascular death, MI, and stroke in patients with a history of MI.
  • Prevention of stent thrombosis after percutaneous coronary intervention (PCI).
  • Used in conjunction with aspirin for optimal antiplatelet therapy.

Directions For Use

Take orally, usually twice daily, with or without food, as directed by your physician. Do not crush or chew tablets.

Benefits

  • Significantly reduces the risk of major adverse cardiovascular events.
  • Faster onset of action compared to some other antiplatelet agents.
  • Provides consistent and potent antiplatelet inhibition.
  • Reduces the incidence of stent thrombosis post-PCI.
  • Proven efficacy in patients with acute coronary syndromes.
  • Helps prevent recurrent heart attacks and strokes.

Side Effects

  • Bleeding (e.g., nosebleeds, bruising, gastrointestinal bleeding)
  • Dyspnea (shortness of breath)
  • Headache
  • Nausea
  • Diarrhea
  • Dizziness
  • Bradycardia (slow heart rate)
  • Hypotension (low blood pressure)
  • Rash
  • Increased uric acid levels
  • Gout
  • Syncope (fainting)

Safety Measures

  • Alcohol - Alcohol can increase the risk of bleeding, especially gastrointestinal bleeding, when taken with ticagrelor. It is advisable to limit or avoid alcohol consumption.
  • Pregnancy - Ticagrelor is generally not recommended during pregnancy unless the potential benefit outweighs the potential risk to the fetus. Consult a doctor.
  • Breastfeeding - It is unknown if ticagrelor is excreted in human milk. A decision should be made whether to discontinue breastfeeding or discontinue the drug, considering the importance of the drug to the mother.
  • Liver - Use with caution in patients with moderate hepatic impairment. Ticagrelor is contraindicated in severe hepatic impairment due to increased exposure and bleeding risk.
  • Kidney - No dose adjustment is generally required for patients with renal impairment, including those on dialysis. However, caution is advised in severe renal impairment.
  • Lung - Ticagrelor can cause dyspnea (shortness of breath), which is usually mild and transient. Patients with pre-existing respiratory conditions should be monitored.
