About

Gemcitabine is a nucleoside analog antimetabolite that exhibits potent antineoplastic activity. It acts by inhibiting DNA synthesis through two primary mechanisms: incorporation into DNA, leading to chain termination, and inhibition of ribonucleotide reductase, which reduces the intracellular concentration of deoxyribonucleotides essential for DNA replication and repair. This dual action makes it highly effective against a broad spectrum of solid tumors.

Administered intravenously, gemcitabine is rapidly metabolized intracellularly to its active diphosphate and triphosphate forms. Its efficacy is dose-dependent, and it is often used in combination with other chemotherapeutic agents or radiation therapy. Careful monitoring of hematological parameters is essential due to its myelosuppressive effects, which are the primary dose-limiting toxicities.

Uses

  • Treatment of pancreatic cancer
  • Management of non-small cell lung cancer (NSCLC)
  • Therapy for breast cancer
  • Treatment of ovarian cancer

Directions For Use

Administer intravenously over 30 minutes. Dosage and schedule are determined by a physician based on the specific cancer type and patient's condition.

Benefits

  • Broad-spectrum efficacy against solid tumors
  • Can improve survival in various cancers
  • Often used in combination regimens
  • Well-established in clinical practice
  • Offers a distinct mechanism of action
  • Contributes to disease control and palliation

Side Effects

  • Myelosuppression (neutropenia, thrombocytopenia, anemia)
  • Nausea and vomiting
  • Fatigue and asthenia
  • Flu-like symptoms
  • Rash
  • Elevated liver enzymes
  • Proteinuria/hematuria
  • Dyspnea
  • Peripheral edema
  • Alopecia (mild)
  • Diarrhea
  • Infection risk

Safety Measures

  • Alcohol - Avoid alcohol during gemcitabine treatment as it may exacerbate gastrointestinal side effects and could potentially interact with the medication's metabolism.
  • Pregnancy - Gemcitabine is contraindicated in pregnancy due to its genotoxic and teratogenic potential. Women of childbearing potential must use effective contraception during and for a period after treatment.
  • Breastfeeding - Breastfeeding is contraindicated during gemcitabine treatment due to the potential for serious adverse reactions in the breastfed infant.
  • Liver - Use with caution in patients with pre-existing liver impairment. Monitor liver function tests closely, as hepatic dysfunction can affect drug metabolism and increase toxicity.
  • Kidney - Gemcitabine is primarily renally excreted. Use with caution in patients with renal impairment, and monitor kidney function. Dose adjustments may be necessary in severe cases.
  • Lung - Pulmonary toxicity, including interstitial pneumonitis and acute respiratory distress syndrome (ARDS), is a rare but serious side effect. Monitor for new or worsening respiratory symptoms.
