About

Saxagliptin and Metformin is a combination therapy indicated for the management of type 2 diabetes mellitus. Saxagliptin is a DPP-4 inhibitor that works by increasing the levels of active incretin hormones, which in turn enhance glucose-dependent insulin secretion and suppress glucagon release from the pancreas. Metformin, a biguanide, reduces glucose production by the liver, decreases glucose absorption from the intestine, and improves insulin sensitivity in peripheral tissues. This synergistic action targets multiple pathophysiological defects in type 2 diabetes, leading to improved glycemic control. The combination is particularly useful for patients who have not achieved adequate glycemic targets with metformin monotherapy or who require additional glucose-lowering effects.

Uses

  • Treatment of type 2 diabetes mellitus.
  • Improvement of glycemic control.
  • When diet and exercise are insufficient.
  • As an add-on to other antidiabetic agents.

Directions For Use

Take this medication orally, usually twice daily with meals, or as directed by your healthcare provider. Swallow the tablets whole.

Benefits

  • Provides comprehensive glycemic control.
  • Significantly lowers HbA1c levels.
  • Low risk of hypoglycemia when used alone.
  • Generally weight-neutral.
  • Improves both fasting and postprandial glucose.
  • Convenient fixed-dose combination.

Side Effects

  • Upper respiratory tract infection
  • Urinary tract infection
  • Headache
  • Nasopharyngitis
  • Diarrhea
  • Nausea
  • Abdominal pain
  • Hypoglycemia (when combined with sulfonylureas/insulin)
  • Lactic acidosis (rare, due to metformin)
  • Pancreatitis (rare, due to saxagliptin)
  • Joint pain (arthralgia)
  • Vitamin B12 deficiency (long-term metformin use)

Safety Measures

  • Alcohol - Avoid excessive alcohol intake, especially with metformin, due to an increased risk of lactic acidosis.
  • Pregnancy - Not recommended during pregnancy. Consult your doctor, as alternative treatments may be safer.
  • Breastfeeding - Metformin is excreted in breast milk; saxagliptin's excretion is unknown. Use with caution; consult a healthcare professional.
  • Liver - Contraindicated in severe liver impairment due to the metformin component. Saxagliptin requires caution.
  • Kidney - Contraindicated in severe kidney impairment (eGFR <30 mL/min/1.73m²) due to metformin. Dosage adjustment needed for moderate impairment.
  • Lung - Generally safe for use in patients with lung conditions; however, severe respiratory failure can increase lactic acidosis risk with metformin.
