About

This medication combines Empagliflozin, a sodium-glucose co-transporter 2 (SGLT2) inhibitor, and Metformin, a biguanide. Empagliflozin works by inhibiting SGLT2 in the kidneys, leading to increased glucose excretion in the urine, thereby lowering blood glucose levels and offering cardiovascular and renal protective effects. Metformin primarily reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. The synergistic action of these two agents provides comprehensive glycemic control for adults with type 2 diabetes, addressing different pathophysiological pathways involved in the disease. This combination is particularly effective for patients who require additional glycemic control beyond what can be achieved with monotherapy, helping to improve HbA1c, fasting plasma glucose, and postprandial glucose levels.

Uses

  • Management of type 2 diabetes mellitus in adults.
  • To improve glycemic control when diet and exercise are insufficient.
  • As an add-on therapy to other antidiabetic medications.
  • To reduce the risk of cardiovascular death in patients with type 2 diabetes and established cardiovascular disease.

Directions For Use

Take orally, usually twice daily with meals, or as directed by your physician.

Benefits

  • Provides robust blood sugar control.
  • Reduces HbA1c levels significantly.
  • Offers cardiovascular and renal protective benefits.
  • Supports weight management.
  • Low risk of hypoglycemia when used alone.
  • Addresses multiple pathways of glucose dysregulation.

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal discomfort
  • Metallic taste in mouth
  • Urinary tract infections
  • Genital yeast infections
  • Increased urination
  • Headache
  • Hypoglycemia (when used with insulin or sulfonylureas)
  • Lactic acidosis (rare, but serious)
  • Vitamin B12 deficiency (with long-term use)
  • Ketoacidosis (rare)

Safety Measures

  • Alcohol - Avoid excessive alcohol intake, as it can increase the risk of lactic acidosis, especially when combined with metformin.
  • Pregnancy - Not recommended during pregnancy. Consult your doctor immediately if you become pregnant or plan to conceive.
  • Breastfeeding - Metformin passes into breast milk; Empagliflozin's presence is unknown. Consult your doctor to weigh risks and benefits.
  • Liver - Use with caution in patients with hepatic impairment. Metformin is contraindicated in severe liver disease due to increased risk of lactic acidosis.
  • Kidney - Dose adjustment is required based on kidney function. Metformin is contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²).
  • Lung - Use with caution in patients with severe respiratory insufficiency, as it may increase the risk of lactic acidosis due to hypoxia.
