Pregabalin and Nortriptyline is a synergistic combination medication primarily indicated for the comprehensive management of various chronic neuropathic pain conditions, including diabetic neuropathy, postherpetic neuralgia, and certain forms of fibromyalgia. Pregabalin, an alpha-2-delta ligand, exerts its analgesic effects by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the influx of calcium ions, subsequently reducing the release of several excitatory neurotransmitters, thereby dampening hyperexcitable nerve activity responsible for neuropathic pain. Nortriptyline, a tricyclic antidepressant (TCA), contributes to pain relief by inhibiting the reuptake of norepinephrine and, to a lesser extent, serotonin in the synaptic cleft.
