About

OXA (Oxaliplatin) is a platinum-based antineoplastic agent widely used in cancer chemotherapy. It belongs to the third generation of platinum compounds and exerts its cytotoxic effects by forming DNA adducts, leading to inter- and intra-strand cross-links. This DNA damage inhibits DNA replication and transcription, ultimately triggering apoptosis in cancer cells. Oxaliplatin is administered intravenously and is particularly effective against colorectal cancer, often used in combination with fluoropyrimidines.

Unlike cisplatin and carboplatin, oxaliplatin's primary dose-limiting toxicity is peripheral neuropathy, rather than nephrotoxicity or ototoxicity. Its unique mechanism of action and side effect profile make it a valuable component in multi-drug regimens for advanced malignancies. Careful monitoring of neurological symptoms is crucial during treatment to manage and mitigate potential long-term effects.

Uses

  • Adjuvant treatment of stage III colon cancer
  • Treatment of advanced colorectal cancer
  • Gastric cancer (off-label)
  • Pancreatic cancer (off-label)

Directions For Use

Administered intravenously by a healthcare professional. Dosage and infusion rate are determined by your doctor based on your body surface area and treatment regimen.

Benefits

  • Effective against colorectal cancer
  • Improves disease-free survival in adjuvant settings
  • Often used in combination with other chemotherapies for enhanced efficacy
  • Different toxicity profile compared to other platinum agents
  • Established role in metastatic disease management
  • Can lead to tumor regression and symptom control

Side Effects

  • Peripheral neuropathy (acute and chronic)
  • Nausea and vomiting
  • Fatigue and weakness
  • Diarrhea or constipation
  • Low blood cell counts (anemia, neutropenia, thrombocytopenia)
  • Mouth sores (stomatitis)
  • Loss of appetite
  • Liver enzyme elevations
  • Allergic reactions
  • Taste disturbances
  • Hair thinning
  • Cold sensitivity (pharyngolaryngeal dysesthesia)

Safety Measures

  • Alcohol - Avoid alcohol during treatment as it may exacerbate gastrointestinal side effects and liver toxicity.
  • Pregnancy - Contraindicated in pregnancy due to potential fetal harm. Effective contraception is required for both male and female patients during and for several months after treatment.
  • Breastfeeding - Breastfeeding is not recommended during treatment and for a period after the last dose due to potential harm to the infant.
  • Liver - Use with caution in patients with pre-existing liver impairment; dose adjustments may be necessary. Regular monitoring of liver function is essential.
  • Kidney - Use with caution in patients with pre-existing renal impairment. While less nephrotoxic than cisplatin, kidney function should be monitored.
  • Lung - Monitor for new or worsening pulmonary symptoms, such as interstitial lung disease, though this is a rare side effect.
