About

SITARA DM 500 is a combination medication for the management of Type 2 Diabetes Mellitus, containing Sitagliptin and Metformin. Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that enhances the body's natural ability to lower high blood glucose levels by increasing the active levels of incretin hormones. These hormones stimulate insulin release from the pancreas and decrease glucagon secretion, both in a glucose-dependent manner. Metformin, a biguanide, works primarily by reducing glucose production in the liver, decreasing glucose absorption from the intestine, and improving insulin sensitivity in peripheral tissues. This complementary action of Sitagliptin and Metformin provides comprehensive glycemic control, addressing both impaired insulin secretion and insulin resistance. The combination is effective in achieving target HbA1c levels and is a valuable option for patients whose blood sugar is not adequately controlled with either monotherapy or who require a more robust approach to manage their diabetes and prevent associated complications.

Uses

  • Management of Type 2 Diabetes Mellitus
  • Improved glycemic control in adults
  • Adjunct to diet and exercise
  • Prevention of diabetes-related complications

Directions For Use

Take this medication with meals, usually once or twice daily, as directed by your physician. Swallow the tablet whole with water.

Benefits

  • Provides effective blood sugar control
  • Reduces HbA1c levels significantly
  • Low risk of hypoglycemia (from Sitagliptin)
  • Weight-neutral or modest weight loss (Metformin)
  • Offers a dual mechanism of action
  • Improves pancreatic beta-cell function

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal pain
  • Headache
  • Upper respiratory tract infection
  • Nasopharyngitis
  • Lactic acidosis (rare but serious)
  • Metallic taste
  • Vitamin B12 deficiency
  • Pancreatitis (rare)
  • Joint pain
  • Skin rash

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can increase the risk of hypoglycemia and lactic acidosis, especially with Metformin.
  • Pregnancy - Generally not recommended during pregnancy. Insulin is usually preferred for glycemic control in pregnant women. Consult your doctor.
  • Breastfeeding - Not recommended during breastfeeding as Metformin passes into breast milk and Sitagliptin data is limited. Consult your doctor.
  • Liver - Use with caution in patients with liver conditions. Metformin is contraindicated in severe hepatic impairment due to increased risk of lactic acidosis.
  • Kidney - Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73m²) due to Metformin. Dose adjustment is necessary for moderate impairment for both components.
  • Lung - Use with caution in patients with severe respiratory conditions or acute heart failure, as these can increase the risk of lactic acidosis.
