About

This medication is a fixed-dose combination of Telmisartan, an Angiotensin Receptor Blocker (ARB), and Amlodipine, a dihydropyridine Calcium Channel Blocker (CCB). Telmisartan works by selectively blocking the AT1 receptor, preventing the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure. Amlodipine acts by inhibiting the influx of calcium ions into vascular smooth muscle and myocardial cells, causing peripheral arterial vasodilation and a reduction in peripheral vascular resistance. The synergistic action of these two agents provides comprehensive and effective blood pressure control, often achieving target blood pressure levels when monotherapy is insufficient. This combination is particularly useful for patients requiring more aggressive blood pressure management or those who benefit from the distinct mechanisms of action.

Uses

  • Treatment of essential hypertension.
  • Management of hypertension in patients requiring multiple antihypertensive agents.
  • Reduction of cardiovascular risk in high-risk hypertensive patients.
  • When monotherapy with either telmisartan or amlodipine is inadequate.

Directions For Use

Take orally once daily, with or without food, as directed by your physician. Swallow the tablet whole with water.

Benefits

  • Provides powerful and sustained blood pressure reduction.
  • Combines two distinct mechanisms for comprehensive control.
  • Reduces the risk of stroke, heart attack, and other cardiovascular events.
  • Offers 24-hour blood pressure control.
  • Convenient single-pill regimen improves adherence.
  • Well-tolerated with a favorable side effect profile.

Side Effects

  • Dizziness
  • Headache
  • Peripheral edema (swelling of ankles/feet, due to amlodipine)
  • Fatigue
  • Nausea
  • Flushing (due to amlodipine)
  • Palpitations (due to amlodipine)
  • Hypotension (low blood pressure)
  • Hyperkalemia (less common with combination)
  • Renal dysfunction (telmisartan component)
  • Gingival hyperplasia (rare, amlodipine component)
  • Cough (less common than with ACE inhibitors)

Safety Measures

  • Alcohol - Alcohol can enhance the hypotensive effects of both telmisartan and amlodipine, leading to increased dizziness or lightheadedness. It is advisable to limit or avoid alcohol consumption.
  • Pregnancy - Telmisartan is contraindicated in pregnancy, especially during the second and third trimesters, due to the risk of fetal injury and death. Amlodipine should be used with caution. Discontinue immediately if pregnancy is detected.
  • Breastfeeding - Telmisartan is not recommended during breastfeeding. Amlodipine is excreted in human milk. A decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother.
  • Liver - Use with caution in patients with hepatic impairment. Dosage adjustments may be necessary for both components. Close monitoring of liver function is recommended.
  • Kidney - Use with caution in patients with renal impairment. Regular monitoring of kidney function and serum potassium levels is crucial, as telmisartan can affect renal function.
  • Lung - This combination generally does not directly affect lung function. However, patients with severe lung conditions should be monitored for any systemic effects that might indirectly impact their respiratory status, especially fluid balance.
