About

This medication combines an angiotensin II receptor blocker (ARB), losartan, with a thiazide diuretic, hydrochlorothiazide, offering a powerful approach to managing hypertension. Losartan works by selectively blocking the binding of angiotensin II to the AT1 receptor, leading to effective vasodilation, reduced aldosterone secretion, and a subsequent decrease in blood pressure. Hydrochlorothiazide complements this action by enhancing the excretion of sodium and water through its inhibitory effect on sodium reabsorption in the distal convoluted tubule, further contributing to blood pressure reduction and reducing fluid volume. This synergistic combination is highly effective in treating essential hypertension, particularly in patients whose blood pressure is not adequately controlled with monotherapy. It provides robust and sustained blood pressure control, thereby significantly reducing the risk of cardiovascular events such as stroke and myocardial infarction.

Uses

  • Treatment of essential hypertension.
  • Management of high blood pressure not controlled by monotherapy.
  • Reduction of cardiovascular risk in hypertensive patients.
  • Prevention of stroke in patients with hypertension and left ventricular hypertrophy.

Directions For Use

Take this medication orally, usually once daily, with or without food, as directed by your physician. It is often recommended to take it in the morning.

Benefits

  • Effective blood pressure reduction.
  • Provides sustained blood pressure control.
  • Reduces the risk of stroke and heart attack.
  • Combines two complementary mechanisms of action.
  • Well-tolerated in many patients.
  • Convenient once-daily dosing.

Side Effects

  • Dizziness
  • Fatigue
  • Headache
  • Nausea
  • Diarrhea
  • Cough (less common than ACE inhibitors)
  • Hypotension (low blood pressure)
  • Electrolyte imbalances (e.g., hypokalemia)
  • Increased uric acid levels
  • Muscle cramps
  • Rash
  • Pancreatitis (rare)

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can exacerbate the blood pressure-lowering effect, leading to dizziness or fainting, and may increase the risk of dehydration.
  • Pregnancy - Contraindicated during pregnancy, especially in the second and third trimesters, due to the risk of fetal renal dysfunction and other serious adverse effects.
  • Breastfeeding - Not recommended during breastfeeding as both components may pass into breast milk and could harm the infant.
  • Liver - Use with caution in patients with severe hepatic impairment as losartan metabolism may be altered. Thiazides should be used with caution in liver disease due to electrolyte disturbances.
  • Kidney - Contraindicated in anuria. Use with caution and monitor renal function closely in patients with severe renal impairment, as hydrochlorothiazide efficacy decreases and losartan may worsen renal function.
  • Lung - Generally safe for use in patients with lung conditions. However, caution is advised in patients with a history of gout or asthma, as hydrochlorothiazide can sometimes exacerbate these conditions.
