About

Fenofibrate is a well-established fibric acid derivative primarily indicated for the management of various forms of dyslipidemia, particularly severe hypertriglyceridemia and mixed dyslipidemia characterized by elevated triglycerides and low high-density lipoprotein (HDL) cholesterol. Its therapeutic efficacy stems from its ability to activate peroxisome proliferator-activated receptor alpha (PPARα), a nuclear receptor that regulates lipid metabolism. Activation of PPARα leads to several beneficial changes, including increased synthesis of lipoprotein lipase, an enzyme crucial for breaking down triglycerides, and a reduction in the production of very low-density lipoprotein (VLDL) particles by the liver. Concurrently, it promotes an increase in HDL cholesterol levels. Fenofibrate is a vital treatment option for patients at high risk of pancreatitis due to extremely high triglyceride levels and plays a significant role in improving the overall atherogenic lipid profile, thereby contributing to cardiovascular risk reduction.

Uses

  • Treatment of severe hypertriglyceridemia.
  • Management of primary hypercholesterolemia.
  • Treatment of mixed dyslipidemia.
  • Reduction of cardiovascular risk in patients with type 2 diabetes and dyslipidemia.

Safety Measures

  • Alcohol - Limit alcohol intake as it can increase triglyceride levels and potentially exacerbate liver side effects associated with fenofibrate.
  • Pregnancy - Fenofibrate is generally not recommended during pregnancy due to insufficient data on safety. Consult a doctor to discuss alternatives.
  • Breastfeeding - It is unknown if fenofibrate is excreted in human milk. Breastfeeding is generally not advised while on this medication.
  • Liver - Contraindicated in patients with active liver disease or unexplained persistent liver function abnormalities. Regular monitoring of liver enzymes is essential.
  • Kidney - Contraindicated in patients with severe renal impairment. Dose adjustment is necessary for moderate renal impairment.
  • Lung - Generally safe for use in patients with lung conditions; however, monitor for any unexplained respiratory symptoms, though rare.
