About

Levetiracetam is a widely prescribed antiepileptic drug (AED) indicated for the management of various seizure disorders. While its exact mechanism remains partially elucidated, it is understood to exert its anticonvulsant effects by binding to synaptic vesicle protein 2A (SV2A) in the brain. This interaction is thought to modulate neurotransmitter release, thereby stabilizing neuronal hyperexcitability and reducing the propensity for seizure generation and propagation. The injectable formulation of levetiracetam is particularly advantageous for patients who cannot tolerate oral medication, such as those experiencing acute seizure episodes or requiring rapid seizure control in critical care settings. It serves as both monotherapy and adjunctive therapy for partial-onset seizures, myoclonic seizures in juvenile myoclonic epilepsy, and primary generalized tonic-clonic seizures, offering a broad spectrum of efficacy with a generally favorable safety profile.

Uses

  • Treatment of partial-onset seizures with or without secondary generalization.
  • Adjunctive therapy for myoclonic seizures in juvenile myoclonic epilepsy.
  • Adjunctive therapy for primary generalized tonic-clonic seizures.
  • Management of status epilepticus in emergency settings.

Directions For Use

Administer intravenously as a slow infusion over 15 minutes, as prescribed by a physician. Dosage is individualized based on patient response.

Benefits

  • Effective in controlling various seizure types.
  • Rapid onset of action with intravenous administration.
  • Generally well-tolerated with a favorable safety profile.
  • Can be used as monotherapy or adjunctive therapy.
  • Minimal drug-drug interactions compared to other AEDs.
  • Improves quality of life by reducing seizure frequency.

Side Effects

  • Somnolence
  • Dizziness
  • Asthenia
  • Headache
  • Irritability
  • Nasal congestion
  • Anorexia
  • Infection
  • Behavioral changes (agitation, aggression)
  • Injection site pain
  • Thrombocytopenia (rare)
  • Stevens-Johnson syndrome (rare)

Safety Measures

  • Alcohol - Avoid alcohol consumption as it can increase central nervous system depression and exacerbate side effects like dizziness and drowsiness.
  • Pregnancy - Use during pregnancy should be carefully considered. While generally considered safer than some other AEDs, consult a doctor to assess risks and benefits.
  • Breastfeeding - Levetiracetam is excreted into breast milk. Breastfeeding is generally not recommended; discuss with a healthcare provider.
  • Liver - No specific dose adjustment is typically required for mild to moderate liver impairment. Use with caution in severe cases.
  • Kidney - Dose adjustment is necessary in patients with renal impairment, as the drug is primarily excreted by the kidneys.
  • Lung - Generally safe for use in patients with lung conditions; however, monitor for any signs of respiratory depression, especially with concomitant CNS depressants.
