About

GLIPTAGREAT M 1000 is a combination oral antidiabetic medication used for the management of Type 2 Diabetes Mellitus. It typically contains a dipeptidyl peptidase-4 (DPP-4) inhibitor, such as vildagliptin or sitagliptin, and metformin hydrochloride. The DPP-4 inhibitor works by increasing the levels of incretin hormones, which stimulate insulin release from the pancreas and suppress glucagon secretion in a glucose-dependent manner, thereby lowering blood glucose. Metformin, a biguanide, primarily reduces hepatic glucose production and improves insulin sensitivity in peripheral tissues, leading to decreased glucose absorption from the gastrointestinal tract. This dual mechanism of action provides comprehensive glycemic control, helping to reduce both fasting and post-meal blood sugar levels. It is prescribed as an adjunct to diet and exercise when monotherapy with either component is insufficient to achieve adequate glycemic targets.

Uses

  • Management of Type 2 Diabetes Mellitus.
  • Improvement of glycemic control.
  • Used as an adjunct to diet and exercise.
  • When monotherapy with metformin or a gliptin is insufficient.

Directions For Use

Take orally with meals, usually once or twice daily, as directed by your physician.

Benefits

  • Improved blood sugar control.
  • Reduced HbA1c levels.
  • Dual mechanism of action for comprehensive management.
  • Potential for weight neutrality or modest weight loss (Metformin).
  • Convenient fixed-dose combination.
  • Lower risk of hypoglycemia compared to sulfonylureas.

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal pain
  • Headache
  • Upper respiratory tract infection
  • Nasopharyngitis
  • Hypoglycemia (especially with other antidiabetics)
  • Lactic acidosis (rare, Metformin)
  • Pancreatitis (rare, gliptins)
  • Joint pain
  • Skin reactions
  • Vitamin B12 deficiency (long-term Metformin)

Safety Measures

  • Alcohol - Avoid excessive alcohol consumption as it can increase the risk of lactic acidosis, especially with Metformin.
  • Pregnancy - Generally not recommended during pregnancy unless clearly necessary, as safety data is limited. Consult a doctor.
  • Breastfeeding - Not recommended during breastfeeding as components may pass into breast milk. Consult a doctor.
  • Liver - Use with caution in patients with mild to moderate hepatic impairment. Contraindicated in severe liver disease due to increased risk of lactic acidosis.
  • Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²) due to Metformin accumulation and increased risk of lactic acidosis. Dose adjustment may be needed for moderate impairment.
  • Lung - Use with caution in conditions that may cause hypoxia (e.g., severe respiratory failure) as it can increase the risk of lactic acidosis.
