About

This medication is a fixed-dose combination therapy for Type 2 Diabetes Mellitus, comprising vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and metformin, a biguanide. Vildagliptin works by inhibiting the breakdown of incretin hormones, GLP-1 and GIP, which are released in response to food. This leads to increased glucose-dependent insulin secretion from pancreatic beta cells and decreased glucagon secretion from alpha cells, thereby reducing blood glucose levels. Metformin complements this action by reducing hepatic glucose production, decreasing intestinal glucose absorption, and improving peripheral insulin sensitivity. Together, these agents offer a synergistic approach to improve glycemic control, particularly in patients who have not achieved adequate control with metformin monotherapy or other oral antidiabetic agents.

Uses

  • Treatment of Type 2 Diabetes Mellitus.
  • To improve glycemic control.
  • When diet and exercise are insufficient.
  • As an add-on therapy to other antidiabetic agents.

Directions For Use

Take orally, usually once or twice daily, with or after food to minimize gastrointestinal side effects.

  • Low risk of hypoglycemia (vildagliptin).
  • Weight-neutral or modest weight loss (metformin).
  • Improved pancreatic beta-cell function.
  • Reduced hepatic glucose output.
  • Convenient fixed-dose combination.

Side Effects

  • Nausea
  • Diarrhea

  • Skin reactions

Safety Measures

  • Alcohol - Limit alcohol intake due to increased risk of lactic acidosis with metformin.
  • Pregnancy - Not recommended during pregnancy due to insufficient data. Consult your doctor for alternatives.
  • Breastfeeding - Metformin passes into breast milk; vildagliptin data is limited. Use with caution and medical advice.
  • Liver - Contraindicated in patients with hepatic impairment due to vildagliptin component.
  • Kidney - Contraindicated in severe renal impairment due to metformin. Dose adjustment needed for moderate impairment.
  • Lung - Use with caution in severe respiratory conditions that may predispose to hypoxia and lactic acidosis.
