About

This medication is a combination of two oral antidiabetic agents: Metformin and Glimepiride. Metformin, a biguanide, primarily reduces glucose production by the liver and improves insulin sensitivity in peripheral tissues. Glimepiride, a sulfonylurea, works by stimulating the beta cells in the pancreas to release more insulin. This dual mechanism of action provides comprehensive glycemic control, targeting both insulin resistance and insufficient insulin secretion.

It is prescribed for adults with type 2 diabetes mellitus when diet, exercise, and monotherapy with either metformin or glimepiride alone do not achieve adequate blood sugar control. The combination helps to achieve lower HbA1c levels and better manage postprandial glucose excursions, contributing to improved long-term health outcomes.

Uses

  • Management of type 2 diabetes mellitus.
  • Improved glycemic control in adults.
  • When monotherapy is insufficient.
  • Adjunct to diet and exercise.

Directions For Use

Take orally with meals, usually once or twice daily, as directed by your healthcare provider. Do not skip meals.

Benefits

  • Provides comprehensive blood sugar control.
  • Targets multiple pathways of glucose regulation.
  • Effectively lowers HbA1c levels.
  • Reduces both fasting and post-meal glucose.
  • Convenient combination therapy.
  • May reduce the number of pills taken daily.

Side Effects

  • Hypoglycemia (low blood sugar)
  • Nausea
  • Diarrhea
  • Abdominal pain
  • Headache
  • Dizziness
  • Weight gain (due to glimepiride)
  • Metallic taste
  • Skin rash
  • Photosensitivity
  • Lactic acidosis (rare, due to metformin)
  • Blood dyscrasias (rare, due to glimepiride)

Safety Measures

  • Alcohol - Avoid alcohol as it can increase the risk of hypoglycemia (due to glimepiride) and lactic acidosis (due to metformin).
  • Pregnancy - Not recommended. Glimepiride is contraindicated in pregnancy, and while metformin may be used, insulin is generally preferred for diabetes management during pregnancy.
  • Breastfeeding - Not recommended. Glimepiride is contraindicated during breastfeeding, and while metformin is generally safe, the combination should be avoided.
  • Liver - Use with extreme caution or avoid in severe liver impairment due to increased risk of hypoglycemia and lactic acidosis. Dose adjustments may be necessary.
  • Kidney - Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustments are crucial for moderate impairment to prevent drug accumulation and side effects.
  • Lung - Patients with severe acute or chronic lung conditions that cause hypoxia should use this combination with caution, as hypoxia can predispose to lactic acidosis.
